Provider Search
Instructions for using the Provider Search
- You can search for providers in the network based on any of the fields provided on the form.
- Entering more than one search criteria will return a smaller list of
entries.
- Each item allows only one entry per field, so you may enter 42103 in the
Zip Code but not 42103,42101.
- If your search returns over 500 matches, you will be instructed to redefine your search criteria.
- You can download a complete directory of all providers in the network. The Provider Search page will not display the complete directory.
