 Kentucky Medicaid

Center Care has formed a partnership with WellCare and with Kentucky Spirit to establish a statewide network for each of these plans as they begin servicing the Kentucky's Medicaid population beginning October 1st. Please see the icons for each Kentucky Spirit and WellCare for corresponding amendments to your Center Care participating provider Agreement. Additional you will find a Provider Data sheet template you can use to send us the latest information about your practice if you have not already done so.
